Parish Church Codes

The first pieces of information you need are your Parish and Church Codes. These are Church of England-assigned reference numbers that are needed to track, and group, responses.

It would also be helpful if you could please copy the Parish and Benefice names as given in these lists into the form – we know these might be a bit different to what you use locally, but consistency will be helpful!
